Ascaris (Roundworm) Antibody Test

The Ascaris (Roundworm) Antibody Test is a targeted blood test for detecting antibodies produced by the immune system in response to Ascaris lumbricoides, the most common intestinal roundworm infection in humans. It is suited to patients with unexplained gastrointestinal symptoms, eosinophilia, or a history of travel to or residence in regions where soil-transmitted helminths are endemic, as well as to clinicians investigating persistent abdominal complaints, nutritional deficiency, or suspected parasitic infection where stool microscopy has been inconclusive.

Ascaris infection can be asymptomatic, but heavier burdens may cause abdominal pain, bloating, nausea, diarrhoea, or signs of malabsorption. In some cases, larval migration through the lungs produces respiratory symptoms, and children in particular may experience impaired growth and cognitive development. Because egg shedding can be intermittent, antibody testing offers a useful adjunct to direct parasitological methods.

The test uses an enzyme-linked immunosorbent assay (ELISA) to detect specific antibodies against Ascaris antigens in a simple blood sample. Antibodies typically become detectable within a few weeks of infection and can persist for months after treatment, so results should always be interpreted alongside clinical history, eosinophil counts, and any stool examination findings.

Why it matters

Ascaris lumbricoides infects an estimated 800 million to 1.2 billion people worldwide, making it one of the most prevalent parasitic infections globally. Chronic infection can lead to iron-deficiency anaemia, vitamin A malabsorption, impaired growth in children, and in rare cases intestinal obstruction or biliary complications. Identifying exposure through antibody testing supports timely treatment, helps prevent household and community transmission, and allows clinicians to rule out roundworm as a cause of persistent or unexplained symptoms.
