Manganese is an essential trace element required for healthy bone formation, cellular antioxidant defence, and normal metabolic function. While small amounts are vital for enzymatic activity across multiple organ systems, elevated concentrations can cause significant toxicity, primarily targeting the central nervous system. This test evaluates circulating manganese levels to screen for toxic occupational or environmental exposure, as well as rare dietary deficiencies.
This assessment is particularly valuable for individuals working in industrial environments with airborne metal dust or fumes, such as welding, metallurgy, mining, chemical manufacturing, and battery fabrication. It is also recommended for patients receiving long-term total parenteral nutrition (intravenous feeding), individuals with severe liver impairment that reduces biliary metal clearance, or patients presenting with unexplained motor or neurological symptoms.

Why it matters
Excess manganese readily crosses the blood-brain barrier and accumulates within the basal ganglia, leading to a condition known as manganism. Symptoms often mimic Parkinson's disease, featuring tremors, muscle rigidity, gait instability, and emotional changes. Because neurological damage from chronic manganese accumulation can become permanent, timely detection enables clinicians to identify toxic exposure early, adjust workplace safety controls, remove exposure sources, and protect long-term neurological function.
