The Histone Antibodies Test detects autoantibodies directed against histone proteins, the basic building blocks of chromatin in cell nuclei. These antibodies are strongly associated with drug-induced lupus erythematosus (DILE), a lupus-like syndrome triggered by certain medications, and can also be found in idiopathic systemic lupus erythematosus (SLE) and other autoimmune connective tissue disorders. This test is typically requested when a patient presents with symptoms suggestive of lupus (such as joint pain, rash, fever, or serositis) and has a history of exposure to medications known to induce autoimmunity, or when differentiating between drug-induced and idiopathic lupus is clinically important. It may also be used as part of a broader autoantibody panel in the investigation of undifferentiated connective tissue disease.

The test is performed using an enzyme-linked immunosorbent assay (ELISA) or similar immunoassay technique to detect and quantify IgG antibodies against histones in a simple blood sample. Results are reported qualitatively or semi-quantitatively, with reference to established cut-off values.

Why it matters
Histone antibodies are a key serological marker for drug-induced lupus, a condition that typically resolves after withdrawal of the offending medication. Identifying these antibodies can help distinguish drug-induced lupus from idiopathic SLE, guiding appropriate management and avoiding unnecessary long-term immunosuppression. In idiopathic SLE, histone antibodies may correlate with disease activity and specific clinical manifestations. Early recognition of drug-induced lupus allows for prompt discontinuation of the causative agent and resolution of symptoms, making this test a valuable tool in the differential diagnosis of lupus-like presentations.
