The FTA-Abs IgM test is a specific treponemal assay used to detect IgM-class antibodies directed against Treponema pallidum, the bacterium that causes syphilis. This test is particularly valuable for identifying early or active infection, including congenital syphilis, and for distinguishing recent from past infection. It is typically requested when initial screening tests are reactive, or when clinical suspicion of early syphilis is high but other tests are inconclusive. The test uses an indirect fluorescent antibody technique to detect IgM antibodies, which are the first immunoglobulins to appear after infection and generally indicate an active or recent immune response.

Biomarkers
- Treponema pallidum IgM antibodies (FTA-Abs IgM): Detects IgM antibodies specific to the syphilis bacterium, suggesting current or recent infection.

Why it matters
Identifying IgM antibodies against Treponema pallidum helps clinicians confirm early or active syphilis, enabling timely treatment and reducing the risk of transmission and long-term complications. This test is especially important in pregnancy, where early detection can prevent congenital syphilis, and in individuals with symptoms or a history of exposure.
