Epstein-Barr Virus (EBV) Viral Capsid Antigen IgM
Know if your EBV infection is current or past.
Epstein-Barr Virus (EBV) Viral Capsid Antigen IgM is a blood test used to detect recent or acute infection with the Epstein-Barr virus. EBV is a very common virus that infects most people at some point in their lives. It is the primary cause of infectious mononucleosis (often called glandular fever) and is associated with a range of other conditions, from mild flu-like illness to more serious complications. This test is particularly useful for patients presenting with symptoms such as persistent fatigue, fever, sore throat, and swollen lymph nodes, where the doctor needs to confirm whether EBV is the cause. It is also used to investigate unusual immune responses or to rule out other infections with similar symptoms.
The test works by detecting IgM antibodies, which are the first type of antibody the immune system produces in response to a new infection. A positive IgM result generally indicates a recent or current EBV infection. Because IgM antibodies can persist for several weeks to months after the initial infection, the test is most informative when interpreted alongside the patient's clinical history and other EBV tests, such as Viral Capsid Antigen IgG and Epstein-Barr Nuclear Antigen (EBNA) IgG. Your doctor will use the pattern of these results to determine whether the infection is acute, recent, or in the past.

Why it matters
Identifying an acute EBV infection helps doctors provide appropriate advice, rule out other causes of your symptoms, and monitor for potential complications. For most people, EBV infection is self-limiting, but in some cases it can lead to prolonged fatigue or more serious issues. Knowing your EBV status can bring clarity and guide next steps in your care.
