The Epstein-Barr Virus (EBV) Viral Capsid Antigen IgG Test is a single-analyte blood test used to detect IgG antibodies directed against the viral capsid antigen (VCA) of Epstein-Barr virus. It is typically requested when a clinician needs to establish whether a person has been exposed to EBV in the past, or as part of a broader panel investigating a suspected current or recent infection. The test is suitable for adults and children where EBV exposure or immune status is clinically relevant, including those presenting with prolonged fatigue, fever, sore throat, or lymphadenopathy, and for individuals in whom a mononucleosis-like illness is being considered.

  • Epstein-Barr Virus (EBV) Viral Capsid Antigen IgG: Detects IgG-class antibodies against the EBV viral capsid antigen. A positive result generally indicates past exposure to EBV and the presence of an established antibody response. This marker is commonly interpreted alongside other EBV serology markers (such as VCA IgM, EBNA-1 IgG, and heterophile antibody tests) to help distinguish between acute infection, past infection, and non-specific or cross-reactive antibody findings.

Why it matters

Epstein-Barr virus is a very common herpesvirus, with the majority of adults worldwide showing evidence of past infection. While many infections are mild or asymptomatic, EBV can cause infectious mononucleosis and has been associated with a range of clinical presentations, including persistent fatigue and certain lymphoproliferative and autoimmune conditions. Establishing EBV exposure status helps clinicians interpret symptoms correctly, avoid unnecessary investigations, and make informed decisions about further testing or management. Because EBV antibody patterns change over time, this test is most informative when considered alongside the clinical picture and, where appropriate, additional EBV serology.
