The Endomysial Antibodies IgA (EMA) test is a highly specific blood test used to support the diagnosis of coeliac disease, an autoimmune condition triggered by gluten. It is most appropriate for individuals experiencing persistent digestive symptoms such as bloating, diarrhoea, or unexplained weight loss, as well as those with non-gastrointestinal signs like iron deficiency anaemia, fatigue, or dermatitis herpetiformis. It may also be requested for individuals with a family history of coeliac disease or other autoimmune conditions such as type 1 diabetes or thyroid disease. The test works by detecting IgA class autoantibodies directed against endomysium, a connective tissue component of smooth muscle. The presence of these antibodies strongly correlates with coeliac disease and reflects the immune response occurring in the small intestine. The analytical method used is indirect immunofluorescence (IIF) on primate tissue or human umbilical cord substrate, a well-established technique for identifying EMA. It is important to note that this test requires a gluten-containing diet prior to sampling to avoid false-negative results. Why it matters
Endomysial antibodies are considered the most specific serological marker for coeliac disease, with a high positive predictive value. A positive result, especially in the presence of compatible symptoms, makes coeliac disease very likely and usually warrants referral for confirmatory duodenal biopsy. However, because EMA testing can be less sensitive than tissue transglutaminase (tTG) in some cases, it is often used alongside tTG IgA and total IgA to improve diagnostic accuracy. Identifying coeliac disease is crucial because strict adherence to a gluten-free diet can resolve symptoms, heal intestinal damage, and prevent long-term complications such as osteoporosis, infertility, and certain malignancies. This test provides a reliable, non-invasive first step in the diagnostic pathway, helping to guide timely and appropriate clinical management.
