The Centromere Antibodies (ACA) Test is a targeted blood investigation used to detect autoantibodies directed against centromere proteins. These antibodies are strongly associated with the limited cutaneous form of systemic sclerosis (formerly known as CREST syndrome) and can also be found in primary biliary cholangitis and other autoimmune conditions. The test is typically requested when a patient presents with features suggestive of scleroderma spectrum disease, such as Raynaud's phenomenon, skin thickening, calcinosis, oesophageal dysmotility, or telangiectasia, and it helps clinicians distinguish between different autoimmune connective tissue diseases.

This test is suitable for adults and young people being investigated for suspected systemic sclerosis or related autoimmune disorders. It is also used to monitor disease progression and to guide prognosis, as centromere antibodies are associated with a more favourable outcome in systemic sclerosis compared with other autoantibody specificities. A simple blood sample is collected by a trained phlebotomist, and the sample is analysed in the laboratory using an immunoassay technique to detect and quantify centromere antibodies.

Identifying centromere antibodies provides crucial diagnostic and prognostic information. A positive result supports a diagnosis of limited cutaneous systemic sclerosis, helps differentiate it from other scleroderma variants and connective tissue diseases, and informs clinical monitoring for associated complications such as pulmonary arterial hypertension and oesophageal involvement. Early detection allows for timely multidisciplinary care and targeted management, improving long-term outcomes for patients with these complex autoimmune conditions.
