The Soluble Transferrin Receptor (sTfR) test is a specialised blood test used to assess iron status, particularly in situations where standard iron markers can be misleading. It is especially valuable for distinguishing between iron deficiency anaemia and the anaemia of chronic disease (also known as anaemia of inflammation), a distinction that is often difficult to make using conventional iron studies alone.

This test is for patients who have anaemia or low haemoglobin alongside a long-term health condition such as chronic kidney disease, rheumatoid arthritis, inflammatory bowel disease, or cancer, where inflammation can distort the results of ferritin and other iron markers. It is also useful for assessing iron status in individuals with suspected iron deficiency when ferritin results are borderline or difficult to interpret.

Marker included
- Soluble transferrin receptor (sTfR): a protein shed from the surface of developing red blood cells. Its level reflects the body's demand for iron and the rate of red cell production. Unlike ferritin, sTfR is not significantly affected by inflammation, making it a reliable indicator of iron deficiency even in the presence of chronic illness.

Why it matters
Accurate identification of iron deficiency is essential for appropriate treatment. In patients with chronic inflammation, ferritin can appear normal or elevated despite true iron deficiency, leading to under-treatment. The sTfR test provides a clearer picture, helping clinicians decide whether iron supplementation is needed and avoiding unnecessary iron therapy in those with adequate stores. This targeted approach supports better management of anaemia and underlying conditions.

No special preparation required.

For blood tests, we recommend drinking plenty of water beforehand to ensure smooth sample collection. If you are taking any regular medication, continue to do so unless advised otherwise by your doctor.

Important Clinical Information:

This test is a screening tool to monitor and support your overall health. It does not replace your GP or consultant.

Always discuss your test results, especially any abnormal results, with your GP, who can interpret them in the context of your medical history and overall health.
