Prostatic Acid Phosphatase (PAP) Blood Test
A legacy marker for prostate health and bone involvement.
The Prostatic Acid Phosphatase (PAP) blood test measures the level of prostatic acid phosphatase, an enzyme produced primarily by the prostate gland. Historically, PAP was one of the first biochemical markers used in prostate cancer assessment, and it remains a useful adjunct in specific clinical scenarios. This test is typically requested by clinicians investigating prostate disease, particularly when there is a need to assess for advanced or metastatic prostate cancer, or to monitor disease progression in certain contexts. It may also be used alongside other investigations when prostate cancer is suspected or known, especially where bone involvement is a concern.
What the test measures
- Prostatic acid phosphatase (PAP): An enzyme produced by the prostate gland. Elevated levels can occur in prostate cancer, particularly when the disease has spread beyond the prostate, as well as in some non-cancerous prostate conditions. Your clinician will interpret the result in the context of your medical history, physical examination, and other test findings.
Who this test is for
This test may be appropriate for individuals who are being investigated for prostate cancer, those with a known diagnosis of prostate cancer where additional information about disease status is needed, or in specific monitoring situations determined by a specialist. It is not a general screening test for prostate cancer and is not a substitute for other established assessments such as prostate-specific antigen (PSA) testing or digital rectal examination. Your doctor will advise whether PAP testing is suitable for your individual circumstances.
Why it matters
Understanding PAP levels can provide valuable information about prostate health and, in certain clinical settings, help your doctor assess whether prostate cancer may be more advanced or involving other areas. This can support informed decisions about further investigation and management. As with all laboratory tests, results are interpreted alongside your clinical picture, and your doctor will discuss what the findings mean for you.

No special preparation required.
For blood tests, we recommend drinking plenty of water beforehand to ensure smooth sample collection. If you are taking any regular medication, continue to do so unless advised otherwise by your doctor.
Important Clinical Information:
This test is a screening tool to monitor and support your overall health. It does not replace your GP or consultant.
Always discuss your test results, especially any abnormal results, with your GP, who can interpret them in the context of your medical history and overall health.
