Islet Cell Antibodies (ICA) Test
Detect autoimmune drivers of beta-cell destruction early.
The Islet Cell Antibodies (ICA) test is a specialised blood test used to detect autoantibodies directed against the insulin-producing beta cells of the pancreas. It is primarily requested for individuals with newly diagnosed type 1 diabetes, those with atypical or slowly progressive diabetes presentations, and family members of people with type 1 diabetes who may be at elevated risk. It is also used to help distinguish type 1 diabetes from type 2 diabetes and other forms of diabetes when the clinical picture is unclear.
The test is performed using a simple blood sample, with analysis typically carried out by indirect immunofluorescence or a validated immunoassay platform. Results are interpreted alongside the patient's clinical history, glucose control, and other diabetes-related autoantibodies such as GAD, IA-2, and ZnT8.

Identifying islet cell autoantibodies provides valuable insight into the underlying autoimmune process driving beta-cell loss. Early detection can support accurate classification of diabetes, guide appropriate treatment decisions, and help identify individuals who may benefit from closer monitoring or emerging immunomodulatory therapies. For family members of people with type 1 diabetes, ICA testing can also inform risk assessment and screening strategies.
