The main function of the PSA is to liquefy semen, allowing sperm to swim freely. PSA level in the bloodstream may be elevated by one or both of the following 2 conditions : 1 . Prostate enlargement As men age, there is an increase in the production of dihydrotestosterone (DHT), a hormone derived from testosterone. DHT stimulates the growth of prostate cells, leading to enlargement over time (benign prostatic hyperplasia, BPH) which increases in the level of PSA in the bloodstream. Symptoms include: -- increased frequency of urination -- weak urine flow -- difficulty initiating or stopping urination -- feeling of incomplete bladder emptying -- blood in semen -- discomfort in the pelvic area -- erectile dysfunction 2. Prostate cancer Prostate cancer is a malignant condition characterized by the abnormal growth of prostate cells and can also lead to elevated PSA level in the blood stream. As prostate cancer also causes the prostate to swell, the symptoms are similar to BHP. The biomarkers in this test assess prostate health by measuring PSA level in the bloodstream 1. tPSA: Measures the total PSA is the sum of free PSA and bound PSA. High levels of total PSA can be a sign of prostate cancer, but it can also be caused by BPH. 2. fPSA: Measures a form of PSA that is not bound to other proteins. High levels of free PSA can be a more specific sign of prostate cancer. 3. PSA ratio: It is the ratio of fPSA to tPSA expressed as a percentage and is used to provide additional information when interpreting PSA test results. A lower PSA ratio indicates a higher likelihood of prostate cancer. It suggests that a smaller proportion of PSA in the blood is in the free form and not bound to other substances.
